HOME TEAM MARYLAND

Resources for Independence is part of the Maryland Home Team. "Home Team" workers help people in nursing homes find suitable housing and support services in the community to meet their needs. Housing Transition Specialists meet with nursing home residents and their family/friends, and nursing home staff to share information about community living options. This includes providing listings of affordable, accessible housing and community support resources. Additionally, Housing Transition Specialists have access to specific funds to assist individuals with security and utility deposits, environmental modifications, and obtaining necessary furnishings.
